*If you wish to apply in more than ONE subject area, you must submit a separate, complete application package for each program.
To fill out the application form, be sure to have the following information on hand before you begin:
- Your personal information, including name, address, e-mail address, and social security number
- Your parent's personal information, including their address and e-mail address
- Your school's complete address, phone and fax number, and your Principal's name
- The full name, address and phone numbers of two adults (a teacher, private instructor, etc.) who would be willing to recommend you
- Your (California) State Senator and State Assembly Member (you can get this info from your county library or county clerk, or try on the Web at California State Legislature information).
